Student Forms and Enrollment

When you register your child ALL paperwork must be completed by your child’s first day. The only exception is the child’s health assessment. The director will discuss the services that will be provided and answer any questions you might have.

The emergency contact form must be completed and updated every 6 months. A child health assessment must be completed within the first 30 days of enrollment and then updated yearly.

The Getting to know you meeting form must be filled out.

The agreement form is a legal contract with Playtime Learning Center. Once your weekly fee is determined you are responsible for this amount regardless of your child’s attendance and the center’s closings. If you choose to leave the program you must give 1 weeks notice. Please take the time to read this form so there are no misunderstandings.

All forms are mandatory requirements of the Department of Public Welfare and there will be no exceptions.
